Licensed Practical Nurse (LPN)

Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care and nursing scope of practice, closely monitoring progress and recovery. Assess and document patient conditions, report changes, administer medications including insulin and IV/fluids, inspect and care for wounds, perform routine nursing duties, and collaborate with providers and other health professionals to ensure optimal patient outcomes. Requires graduation from a practical nursing program and a current LPN/LVN license in the state of employment, plus CPR/AED/BLS/First Aid certification. Location: Latrobe, PA area with in-person, field-based home health care responsibilities. Roles offer full-time, part-time, or PRN scheduling and daily-pay option; no overtime required.
Required Qualifications
- Graduate of a practical nursing program for LPN/LVN
- Current license as an LPN/LVN in the state where the position is located
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred, not required
