Licensed Practical Nurse (LPN)

Job Summary
Provide in-home direct patient care per the provider's care plan and nursing scope; monitor vitals and patient progress; administer medications including insulin and IV fluids; inspect and care for wounds, change dressings, and assist with personal hygiene; assess patients for injuries or complications and deliver appropriate treatments; document changes thoroughly and report to the care team; collaborate with providers, aides, and allied health professionals to ensure optimal patient outcomes.
Required Qualifications
- Graduate of a practical nursing program for LPN/LVN
- Current unrestricted LPN/LVN license in the state of employment
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites/work locations
- Practical trach and/or ventilator experience preferred, not required
