Licensed Practical Nurse (LPN)

Provide in-home direct patient care per the provider's plan of care and nursing scope; monitor progress and report changes (vital signs, wound care, glucose, etc.); administer medications including insulin and IV fluids with thorough documentation; inspect and care for wounds and assist with personal hygiene; assess for injuries or infections and deliver appropriate treatments; collaborate with providers and other health team members to ensure optimal patient outcomes.
