Licensed Practical Nurse (LPN)

As an Licensed Practical Nurse (LPN), you will provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery. You will assess and document patient conditions and report changes as needed (vitals, blood/stool/urine, glucose, etc.), administer medications including insulin and IV/fluids, inspect and care for wounds, assess for further injuries or infections, and collaborate with providers, aides, and other health professionals to ensure exceptional patient outcomes. Qualifications include graduation from a practical nursing program, current LPN license in the state, and CPR/AED/BLS/First Aid certification. Reliable transportation is required to travel to care sites. Trach/ventilator experience is preferred but not required.
