Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care and nursing scope of practice; monitor progress, document vitals and patient conditions; administer medications including insulin and IV/fluids; inspect and care for wounds and assist with personal hygiene; assess for injuries or infections and deliver appropriate treatments; collaborate with providers and the care team to ensure optimal patient outcomes.
Required Qualifications
- Graduate of a practical nursing program for LPN/LVN
- Current license as LPN/LVN in the state of position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
