Job Summary
In-home Licensed Practical Nurse (LPN) providing direct patient care per the prescribed plan of care, including vitals monitoring, medication administration (including insulin and IV/fluids), wound care, and patient hygiene. Collaborates with providers and care team to ensure optimal patient outcomes. Requires active LPN/LVN license in the state of assignment (PA), CPR/AED/BLS/First Aid certification, and reliable transportation for travel to care sites. Flexible scheduling options available (Full-time, Part-time, PRN) with no overtime requirement. Daily patient-focused care in home settings across Greensburg and surrounding areas.
Required Qualifications
- Educational: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as an LPN/LVN in the state position is located
- Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred, not required
