Job Summary
Licensed Practical Nurse (LPN) in-home care: Provide in-home direct patient care according to the provider's plan of care, closely monitor progress, administer medications (including insulin) and IV fluids, inspect and care for wounds, assess patient conditions, and collaborate with other health professionals to ensure outcomes. Requires LPN/LVN nursing education, a current unrestricted LPN license in the state of employment, and CPR/AED/BLS/First Aid certification. Must have reliable transportation to care sites. Trach and/or ventilator experience is preferred but not required. Flexible full-time or part-time scheduling, with focus on one-on-one patient care and a patient-centric approach. Required Qualifications
- Educational: Graduate of a practical nursing program for LPN/LVN
- Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state of position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
