Licensed Practical Nurse (LPN)

Provide in-home direct patient care according to the provider's prescribed plan of care while monitoring progress and recovery. Assess and document patient conditions including vitals, blood work, and glucose levels, then report changes as needed. Administer medications, insulin, and IV fluids with thorough documentation. Inspect patients for injuries, infections, or bedsores, deliver appropriate treatments, and assist with personal hygiene and wound care. Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes. This role includes one-on-one patient care with flexible full-time, part-time, or PRN scheduling.
