Job Summary
Provide direct patient care in accordance with the Nurse Practice Act, focusing on end-of-life support and implementing individualized plans of care under RN supervision. Deliver effective pain and symptom management while educating patients and families on disease progression and comfort measures. Document care accurately in the EMR and communicate changes to the interdisciplinary team, including RNs, social workers, and chaplains. Follow clinical standards, maintain equipment, and participate in quality improvement initiatives.
