Job Summary
Provide skilled nursing care to patients as directed by the IDG plan of care, including personal care, vital signs, and medication administration. Document care to agency standards using the A-I-R protocol and reinforce patient and family teaching on disease processes and self-care techniques. Assist the physician and registered nurse in specialized procedures while eliciting complaints or unusual occurrences. Demonstrate proper sharps disposal, hand hygiene, and use of personal protective equipment. Communicate current patient health status to appropriate health professionals and participate in quality improvement programs. Schedule includes 7am-3pm, 8am-4pm, or 9am-5pm Monday-Friday with holiday rotation, weekend coverage, and back-up visits as needed.
Required Qualifications
- current LPN licensure in the State of New York
- Valid Driver's License, free of serious violations
- vehicle required for use when on duty or on-call
- 1 year of nursing practice as a Licensed Practical Nurse preferred
