Job Summary
Licensed Practical Nurse (LPN) for Hospice Admissions, part-time (20 hours/week) in Savannah, GA. Works under RN supervision to provide nursing care to hospice patients and families, ensuring confidentiality and alignment with hospice philosophy. Requires GA LPN license and a minimum of two years of hospice, oncology, med-surg, or home-health experience, CPR certification, valid driver’s license with automobile insurance and reliable transportation. Strong communication and documentation skills, proficiency with Microsoft Office/Outlook, and ability to document patient encounters using a computer. May involve patient visits in a personal vehicle and occasional non-standard hours.
Required Qualifications
- Graduate of an accredited school of Nursing
- current GA LPN License
- Minimum of two years of experience in hospice, oncology, med-surg or home health providing direct patient care
- Current CPR certification
- valid driver’s license, automobile insurance and reliable transportation
- Excellent communication skills (written and verbal) and the ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large along with excellent customer service and documentation skills
- Computer proficiency in Microsoft Office products including Outlook, and database knowledge
