Job Summary
Provide skilled nursing care under the direction of a Registered Nurse while following each patient's individualized plan of care. Monitor patient conditions, recognize changes in health status, and communicate clinical updates to the care team. Assist with symptom management, medication administration, treatments, and patient comfort measures. Complete accurate and timely documentation of patient care and observations within the electronic medical record (EMR). Collaborate with physicians, Registered Nurses, caregivers, and the interdisciplinary team to deliver compassionate, high-quality hospice care. Requires an active LPN license, previous nursing experience, valid driver's license, current CPR certification, and TB testing.
Required Qualifications
- Active Licensed Practical Nurse (LPN) license
- Previous nursing experience
- Valid driver's license
- Reliable transportation
- Current automobile insurance
- Ability to pass a criminal background check
- Ability to pass a Motor Vehicle Record (MVR) screening
- Current CPR certification
- Current TB testing
