Licensed Practical Nurse, LPN - Home Health

Provide direct patient care in accordance with the state Nurse Practice Act, implementing care plans developed by an RN and delivering skilled nursing services including treatments, medication support, and wound care. Monitor patient conditions, report changes to the RN and interdisciplinary team, and educate patients and caregivers on self-care and disease management. Maintain infection control, document care accurately within the EMR, and communicate effectively with physicians and team members. Participate in case conferences, team meetings, and ongoing training while supporting coordinated care across the territory. Requires an active LPN/LVN license, one year of nursing experience preferred, and familiarity with Homecare Homebase.
