Job Summary
Conduct head-to-toe patient assessments, triage urgent clinical situations, and manage acute and post-acute care for clients in the home setting. Perform comprehensive wound care, medication reconciliation, and safety monitoring while documenting visits efficiently. Work independently to make real-time clinical decisions, prioritize urgent visits, and adapt to varied home environments. Requires a current Colorado nursing license, one year of recent adult/geriatric experience, and the ability to travel throughout the coverage area. Required Qualifications
- A current nursing license in good standing in the state of Colorado
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- At least one year of current or recent hands-on medical experience within the past two years with the adult/geriatric population
- Ability to work independently while knowing when to escalate concerns
- Comfortable making real-time clinical decisions without immediate on-site support
- Strong problem-solving skills in unpredictable home environments
- Ability to manage a dynamic visit schedule and prioritize urgent visits
- Efficient with point-of-care documentation and technology
- Strong attention to detail while maintaining productivity expectations
