Job Summary
Provide skilled nursing visits in home health settings under RN delegation; verify/edit Plan of Care, document nursing notes, communicate changes to care team, ensure patient safety, educate patients on self-care, collaborate with RN Case Manager, document medications, and travel to patient homes (up to 50%) while delivering compassionate care.
Required Qualifications
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Valid driver’s license and insurance and reliable transportation
