Job Summary
Licensed Practical Nurse (LPN) to provide in-home nursing care within a home health setting (PRN). Verifies Plan of Care prior to visits and delivers nursing care per RN delegation, physician orders, assessment data, and established guidelines. Integrates patient care goals into visits, documents skilled nursing notes, communicates critical findings to the RN Case Manager and other care team members, reports unsafe conditions, supports patient safety, teaches self-care to patients and families, and documents medication regimens. Collaborates with the RN Case Manager during regular business hours for guidance, and travels to patient homes as needed (up to ~50% travel). Requires state LPN license, 1 year clinical experience, ability to lift up to 50 pounds, and valid driver's license with reliable transportation. Provides professional, respectful communication with patients, families, and agency staff; participates in care planning and coordination to prevent hospitalizations and ER visits. Required Qualifications
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Valid driver’s license and insurance and reliable transportation to perform job tasks
