Licensed Practical Nurse (LPN) - Home Health - Part Time

Provide direct patient care in accordance with the state Nurse Practice Act, implementing care plans developed by the RN and delivering skilled nursing services including treatments, medication support, and wound care. Monitor patient conditions, report changes to the RN and care team, and educate patients and caregivers on self-care and disease management. Maintain infection control, document care accurately within the EMR, and communicate effectively with physicians and interdisciplinary team members. Participate in case conferences, team meetings, and ongoing training while supporting coordinated care across the team. This field-based role involves delivering consistent, high-quality care in patients' homes within an assigned territory under the supervision of an RN, with a structured yet flexible daily schedule.
