Job Summary
Conduct one-on-one patient care and family teaching in the home setting, administering skilled nursing care as prescribed and in accordance with the established plan of care. Perform timely documentation and collaborate with the interdisciplinary team. Requires a current LPN license, CPR certification, and reliable transportation. Pay-per-visit rates range from $25 to $30 per visit.
Required Qualifications
- Graduate of an accredited Licensed Practical Nurse educational program
- Current Licensed Practical Nurse (LPN) license within the state of practice
- Current CPR certification
- Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
- Reliable transportation
- Valid driver's license
- Current automobile insurance
