Licensed Practical Nurse LPN Home Health

Licensed Vocational / Practical Nurse needed to deliver in-home nursing care for patients in home health settings. Responsibilities include verifying the Plan of Care prior to visits and delivering care under RN or physician delegation, incorporating patient goals into nursing visits, communicating findings to the RN Case Manager and other care team members, identifying and reporting unsafe conditions, implementing the care plan to promote patient safety, assisting patients with self-care techniques, suggesting revisions to the Plan of Care, and documenting medication regimens with updates to patient profiles. Requires collaboration with the RN Case Manager during normal business hours, ability to provide quality patient and family interactions, and travel to patient homes (approximately 50% of time). Related duties include maintaining documentation in skilled nursing notes and delivering high-quality care while complying with established standards. EOE. Pay is per visit and contingent on qualifications and internal equity.
