Job Summary
Licensed Vocational/Practical Nurse (LPN) needed for home health at Elara Caring. You will verify and implement the Plan of Care prior to visits, perform nursing visits under RN/physician delegation, document care and medications, communicate changes to the RN Case Manager and other personnel, ensure patient safety, educate patients and caregivers, and collaborate with the RN Case Manager during normal business hours. Requires travel to patient homes (approximately 50%), ability to lift up to 50 pounds, and a valid driver’s license with reliable transportation. Requires graduation from an accredited nursing program and current LPN license in the state; hospice/home health experience preferred.
Required Qualifications
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
- Current State License as a Licensed Vocational / Practical Nurse
- 1 year of experience in a clinical care setting
- Experience in a hospice or home health environment is preferred
- 50% travel required
- Valid driver’s license and insurance and reliable transportation to perform job tasks
