Job Summary
Conduct home health visits to adult clients in Pitt County, administering medications, performing wound care, and monitoring conditions. Execute assigned treatments and procedures while accurately documenting observations, interventions, and evaluations in electronic medical records on a touch pad tablet. Perform client assessments as necessary and report any changes to Clinical and Client Services Managers. Manage case coordination and ensure proper follow-up on doctors' orders. Requires a current North Carolina LPN license with one year of recent experience and the ability to travel to assigned cases.
Required Qualifications
- One year prior clinical experience as a licensed Practical Nurse
- A current license as a Licensed Practice Nurse in the state of North Carolina
- A minimum of one year of recent, verifiable experience as an LPN
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
