Job Summary
Conduct home health visits to adult clients in Cabarrus and Stanly counties, administering medications, wound care, treatments, and procedures while monitoring conditions and reporting changes to managers. Perform client assessments, execute and document doctors' orders, and manage case coordination using electronic medical records on a touch pad tablet. Maintain accurate documentation of observations, interventions, and evaluations. Requires a current North Carolina LPN license, one year of recent verifiable experience, and strong computer skills.
Required Qualifications
- One year prior clinical experience as a licensed Practical Nurse
- A current license as a Licensed Practice Nurse in the state of North Carolina
- A minimum of one year of recent, verifiable experience as an LPN
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
