Job Summary
Provide one-on-one geriatric patient care in clients' homes under the supervising RN, implement and follow treatment plans, accurately document patient progress, and communicate patient status with the supervising RN across the North Palm Beach County territory (Jupiter, Palm Beach Gardens, Riviera Beach, Juno Beach and surrounding areas). Per-visit compensation is offered and field clinicians use Kinnser/WellSky for charting.
Required Qualifications
- Active Florida LPN license
- Current CPR/BLS certification
- Valid driver’s license and reliable transportation
- Kinnser/WellSky experience is a major plus
