Job Summary
Licensed Practical Nurse (LPN) on the Full-Time PM shift (2:30p-11p) in a long-term care setting. Responsibilities include assessing residents, administering medications and treatments, observing plans of care and updating them as needed, documenting in the electronic medical records, coordinating with interdisciplinary care teams, and providing direct nursing care. The role also involves orienting and assisting with training of new team members, ensuring appropriate staffing levels, and communicating residents’ needs to physicians and families. Must hold an active MN LPN license (or be able to obtain on hire) and participate in weekend/holiday rotation with applicable shift differentials.
Required Qualifications
- Active MN LPN license
- Must be able to obtain MN LPN license upon hire
- Long Term Care experience preferred but not required
