Job Summary
Licensed Practical Nurse (LPN) responsible for providing compassionate geriatric care at a rehab/long-term care setting. Focus on building meaningful relationships with residents and staff while delivering skilled nursing duties, including assessments, documentation, and medication management. Exhibit leadership to promote teamwork within the care team, collaborate with nursing and facility staff for coordinated care, and maintain effective interpersonal communication to support a respectful, positive work environment aligned with the C.A.R.E. values (Compassion, Accountability, Relationships, Excellence). Responsibilities emphasize resident well-being, clinical accuracy, and active participation in care planning and quality care delivery. This is a Full-Time role with Day and Night shifts based in Newburgh, IN.
Required Qualifications
- Licensed Practical Nurse license in the state of Indiana
- Proficient medication management skills
- Ability to conduct thorough assessments and accurately document changes in resident condition
- Strong passion for geriatric nursing and commitment to senior care excellence
- Excellent communication and interpersonal skills
