Licensed Practical Nurse (LPN) - Full-Time

Job Summary
LPN needed for in-home care in the Richmond, VA area for full-time assignments. Deliver direct patient care to individuals requiring tracheostomy/ventilator support, following a plan of care developed with RNs. Provide documentation, family/caregiver education, and coordinate home health services with other disciplines (HHA, OT, PT, MSW, ST, Dietician). Responsibilities include preparing equipment for treatments, observing aseptic technique, assisting patients in self-care, and supporting care plans to keep patients safe and comfortable in their homes. Requires CPR and PPD certifications, 1+ year LPN experience, active LPN license, valid driver’s license and reliable transportation; availability for days, evenings, and potential overnights.
