Job Summary
Conduct clinical monitoring and support for residents with Alzheimer's/Dementia, ensuring restorative, support, and preventative measures meet state laws. Oversee self-administered medication management and administer treatments per physician orders while documenting care and reporting resident observations. Provide emotional support, physical comfort, and safety supervision for Care Partners and Home Health Aides. Assist with resident move-ins, relocations, and community marketing efforts. Work full-time on the 2nd shift (3pm-11pm) with every other weekend required.
Required Qualifications
- current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as granted by State Board of Nursing
- willingness to work with the elderly population
- Experience with Alzheimer's/Dementia or behavioral health conditions
- Must possess current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) designation as granted by State Board of Nursing
- Fulfill and maintain continuing education credits as required by state
- Every other weekend is required
