Licensed Practical Nurse (LPN) - Full-Time 3P-11P

Deliver nursing care to patients requiring long-term or rehabilitative care, including admissions, discharge, and transfers. Administer medications per requisitions and collaborate with the interdisciplinary team to provide the best outcomes. Collect patient data, observe conditions, and report abnormal changes to proper personnel while maintaining confidentiality and following infection control policies. Implement the patient plan of care, evaluate responses, and maintain necessary documentation and equipment knowledge. Works under direct supervision in accordance with state-specific regulations and facility policies.
