Job Summary
Licensed Practical Nurse (LPN) needed for full-time at Scott Lake Health and Rehabilitation Center in Lakeland, FL. Responsibilities include admissions, discharge and transfers of residents; medication requisitions with pharmacy and physicians; medication administration; adherence to infection control and center policies; collaboration with the interdisciplinary team to optimize outcomes; identifying and reporting changes in residents’ condition per policies; working under supervision per state Nurse Practice Act; delivering care for long-term and rehabilitative needs; collecting data, observing, and documenting patient/resident status; implementing and evaluating the care plan per state regulations; directing and supervising care by other nursing personnel when applicable; maintaining documentation and equipment knowledge; maintaining patient/resident confidentiality. LPN must be licensed by the state or hold a multistate license and CPR certification; two years of nursing experience preferred.
