Licensed Practical Nurse (LPN) Every Wed & Thurs 11P-7A

Conduct nursing care for residents requiring long-term or rehabilitative care, including medication administration, data collection, and implementing patient plans of care. Manage admissions, discharges, and transfers while collaborating with the interdisciplinary team to provide the best outcomes. Monitor equipment, maintain documentation, and report abnormal changes in residents' conditions to proper personnel. Follow infection control, emergency, and center policies while working under direct supervision in accordance with the state-specific Nurse Practice Act. Maintain knowledge of confidentiality and patient rights.
