Licensed Practical Nurse - LPN

Provide direct patient care including wound care, medication administration, and vital sign monitoring in home settings. Assess and document patient health status, progress, and changes in condition accurately. Administer prescribed medications and treatments in accordance with physician orders. Collaborate with RNs, physicians, and interdisciplinary care teams to ensure continuity of patient care. Educate patients and family members on care plans, medications, and health management techniques. Maintain accurate and up-to-date patient records in compliance with state and federal regulations. Monitor patients for adverse reactions and report significant changes to supervising RN or physician promptly. Active Licensed Practical Nurse (LPN) license in Maryland required; prior home healthcare experience preferred.
