Job Summary
Provide nursing care in the Adult Day Healthcare program in Gainesville, performing health monitoring and nursing services for participants; assist in coordinating admissions and discharges, building participant charts, arranging in-house medical visits and transportation as needed; track and coordinate quarterly care plan meetings with participants and family/caregiver teams; document and maintain records, report trends outside parameters, and provide clinical support and in-service training to staff; educate participants and families on health, medical diagnoses, medications, safe living, emergencies, and available resources; requires graduation from an accredited nursing program and a current Florida LPN license, with at least one year of experience serving individuals with disabilities, aging populations, or Alzheimer's patients; CPR and first aid certification are a plus and can be obtained/renewed after hire; benefits include medical, dental, vision, paid holidays, vacation, sick and personal time, EAP, retirement plan, and work-life balance opportunities.
Required Qualifications
- Graduate of an accredited nursing program
- Current Florida Licensed Practical Nurse License
- At least one year of experience working with individuals with disabilities and special needs and/or the aging population and Alzheimer's patients
- Ability to assist with the transfer of participants to and from chair or wheelchair and assist clients with activities of daily living (toileting, bathing, ambulating, feeding, changing clothes, etc)
