Job Summary
Provide safe, effective nursing care per care plans and physician orders to a group of residents under general supervision. Administer medications and treatments accurately, assess residents and report changes promptly, document care in the EMR, monitor performance and mentor CNAs, ensure infection control and safety compliance, and perform all other duties as assigned. Excellent communication with patients, families, and health-team members; CPR certified; active LPN license; ability to meet physical job requirements.
Required Qualifications
- Active LPN license in state of Wisconsin or licensed in any U.S state and be eligible for Wisconsin licensure by reciprocity. (in good standing)
- CPR certified
- Ability to meet physical and job-related requirements
- Must be English literate and possess excellent written and verbal communication skills
- Preferred: Long-term care or skilled nursing experience
- Clinical experience
