Job Summary
Conduct home health visits by driving to patient homes, obtaining medical histories, administering immunizations and medications, and performing venipuncture. Document assessments, interventions, and test results while monitoring patient progress and recognizing conditions requiring physician attention. Troubleshoot test systems, maintain weekly equipment checks, and interact compassionately with patients, families, and clinical staff. Maintain HIPAA confidentiality and participate in community health screenings and professional development. Requires a Kentucky LPN license, BLS certification, and valid driver's license. Part-time role in Louisville with medical, dental, vision, and 401k benefits.
Required Qualifications
- LPN License
- Blood Withdrawal / Phlebotomy experience
- Work experience with pediatrics
- A minimum of 2 years of LPN experience
- BLS with American Heart Association
- Valid Kentucky Driver's License (minimum of 2 years) and acceptable driving record
- Excellent verbal communication skills
- Ability to lift 50 - 60 lbs
- Must be comfortable driving and making home health visits independently
- Must have weekday and/or weekend availability
Desired Qualifications
- Patient bedside experience preferred (preferred but not required)
- Spanish speaking (preferred but not required)
