Licensed Practical Nurse (LPN)

Admissions, discharge, and transfers of residents; medication requisitions with pharmacy and physicians; and medication administration. Deliver nursing care to patients/residents requiring long-term or rehabilitative care while following infection control, emergency, and center policies. Collaborate with the interdisciplinary team to provide the best outcomes, identifying and reporting abnormal changes in residents' condition to proper personnel. Collect patient data, make observations, and report pertinent information related to care. Implement the patient/resident plan of care according to state-specific regulations and evaluate the resident's response. Maintain knowledge of documentation requirements, equipment maintenance, and confidentiality regarding patient/resident rights. Works under direct supervision in accordance with the state-specific Nurse Practice Act and facility policies.
