Job Summary
Provides care and treatment to patients on maintenance dialysis as an LPN, within the dialysis unit, under the multidisciplinary team. Delivers skilled nursing care, monitors dialysis treatments, assists with water room equipment operation and safety, and ensures compliance with state, CMS/ESRD, and facility policies. Educates patients and families about the dialysis process and self-care, develops and evaluates educational plans, documents in patient records, and coordinates care with Medical Director, Nephrologist, patients, families, and other nursing staff. Demonstrates compassion, teamwork, and adherence to CARE standards, while maintaining confidentiality and a safe care environment.
Required Qualifications
- Active LPN license in the state of practice
- Dialysis experience preferred or CHN/CPDN/CDN certifications are a plus
- Ability to perform essential job functions with applicable acuity and communication skills
