Licensed Practical Nurse (LPN) - Daylight Hours

Provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress and recovery. Administer medications, insulin, and IV/fluids, document thoroughly, inspect and care for wounds or dressings, and assess patient conditions (vital signs, glucose tracking, etc.). Collaborate with providers and care teammates to ensure exceptional patient outcomes; support patients and families with compassionate, patient-centric care delivered in the home setting.
