Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care while closely monitoring progress; assess and document patient conditions and report changes; administer medications including insulin and IV/fluids; inspect and care for wounds and assist with personal hygiene; assess for further injuries or infections and deliver appropriate treatments; collaborate with providers and care team to ensure exceptional patient outcomes; requires current LPN/LVN license in PA, CPR/BLS/First Aid certification, and reliable transportation; in-home Daylight hours position in Renfrew, PA.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license to practice as Licensed Practical Nurse (LPN) in the state of Pennsylvania
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred, not required
