Job Summary
Provide in-home direct patient care as an LPN, including administering medication, insulin, IV fluids, wound care, and regular assessments per the plan of care; document thoroughly, monitor patient conditions (vital signs, glucose, etc.), and collaborate with providers and other health professionals to ensure high-quality patient outcomes. Must hold a current LPN/LVN license in PA, have completed a practical nursing program, and maintain CPR/AED/BLS/First Aid certification; reliable transportation to multiple care sites required.
Required Qualifications
- Graduate of a practical (vocational) nursing program for LPN/LVN
- Current license to practice as Licensed Practical Nurse (LPN) in the state of position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
