Job Summary
Licensed Practical Nurse (LPN) needed for in-home care in the Callery/Fombell, PA area. Provide in-home direct patient care according to the prescribed plan of care, monitor patient progress, administer medications including insulin and IV fluids, perform wound care and dressing changes, document conditions and report changes, and collaborate with providers and other health professionals to achieve optimal patient outcomes. Responsibilities include assessing patients for injuries or infections, managing vital signs, and delivering appropriate treatments while ensuring patient comfort and dignity. Requires an active LPN/LVN license in the state, CPR/BLS/First Aid certification, and reliable transportation to care sites. Flexible scheduling options available (FT/PT/PRN). Required Qualifications
- Educational: Graduate of a practical (vocational) nursing program for LPN/LVN and has a current license as a LPN/LVN in the state position is located
- Licensure: Current unrestricted license to practice as Licensed Practical Nurse (LPN) in the state associated with this position
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites and/or work locations
- Practical trach and/or ventilator experience preferred, not required
