Job Summary
Provide in-home direct patient care according to the provider's prescribed plan of care and nursing scope of practice, including administering medications (including insulin and IV/fluids), assessing and documenting patient conditions (vital signs, wound care, infections), and delivering appropriate treatments. Collaborate with providers, aides, and other health professionals to ensure quality patient outcomes while observing one-on-one patient care and flexible scheduling (including full-time, part-time, and PRN) in a home-health setting. Must maintain active LPN license in PA and hold CPR/AED/BLS/First Aid certifications, with reliable transportation to care sites. Work-life balance is emphasized in a role with daytime hours (Mon-Fri) in Enon Valley and Fombell, PA under Interim HealthCare.
Required Qualifications
- Current LPN/LVN licensure in the state of employment (Pennsylvania)
- Graduate of a practical (vocational) nursing program
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites/work locations
