Job Summary
Administer oral, injectable, intravenous, and topical medications while evaluating complex patient conditions, monitoring pain levels, and documenting treatment responses. Apply advanced vital sign monitoring skills to identify abnormalities in blood pressure, heart rate, respiratory rate, and temperature. Perform routine assessments to monitor patient health, manage access to community resources for medication administration and disease management, and conduct medication reviews to identify potential side effects and drug interactions. Develop and administer mental health assessments to screen for depression and evaluate mood, behavior, and cognitive function. Accurately document patient medical history using electronic health records and interact with patients, families, and healthcare professionals. Requires an active state LPN/LVN license, NCLEX-PN completion, and proficiency with EMR systems including Epic.
Required Qualifications
- Active and in good standing state LVN or LPN license
- Successful completion of the NCLEX-PN (National Council Licensure Examination for Practical Nurses)
- Knowledge of medical terminology and clinical documentation standards
- Experience with phlebotomy, vaccine administration, and point-of-care testing
- Excellent communication and organizational skills
- Experience working in a clinical setting with diverse cultural dynamics among staff and patients
- Proven ability to manage sensitive information ethically and responsibly
- Strong problem-solving, critical thinking, and decision-making skills
- Proficiency with electronic medical record (EMR) systems
- High school diploma or GED
- Graduate of a State approved Practical or Vocational nursing program
