Job Summary
Monitor patient health by checking vital signs, observing condition changes, and reporting concerns to RNs or physicians. Administer prescribed medications, including oral doses, injections, and IV fluids, while documenting all treatments. Provide wound care through cleaning, dressing changes, and infection monitoring. Assist patients with activities of daily living such as bathing, dressing, grooming, eating, and toileting to promote independence. Collaborate with the healthcare team to develop and implement individualized patient care plans and participate in care conferences. Document all care aspects in the electronic health record system. Provide emotional support to patients and families navigating illness or recovery. Maintain safety by adhering to infection control protocols. Hold an active Indiana LPN license, BLS certification, and complete an accredited LPN program. Full-time direct placement in nursing homes, outpatient, or long-term care settings.
Required Qualifications
- Active and unencumbered LPN license in the state of Indiana
- Completion of an accredited LPN program
- Current Basic Life Support (BLS) certification
- Ability to stand, walk, and move throughout the shift
- Ability to lift and assist patients with mobility and transfers
- Ability to perform tasks requiring manual dexterity and physical stamina
Desired Qualifications
- Previous experience in a clinical setting, particularly in long-term care or outpatient facilities
