Licensed Practical Nurse (LPN)- Contract

Monitor patient health by checking vital signs, observing condition changes, and promptly reporting concerns to RNs or physicians. Administer prescribed medications including oral doses, injections, and IV fluids while documenting all treatments. Provide wound care through cleaning, dressing changes, and infection monitoring. Assist patients with activities of daily living such as bathing, dressing, grooming, eating, and toileting to promote independence. Collaborate with the healthcare team to develop and implement individualized patient care plans. Document all care aspects accurately in the electronic health record system. Provide emotional support to patients and families navigating illness or long-term care. Maintain safety by adhering to infection control protocols. Requires an active Indiana LPN license, BLS certification, and physical ability to lift and transfer patients. Full-time direct placement in South Bend, IN.
