Job Summary
Provide skilled nursing care in patients' homes under supervision of a Registered Nurse; perform patient assessments and assist with care planning; administer medications and treatments as ordered; educate patients and caregivers on disease management and care plans; monitor patient condition and report changes to supervising RN; collaborate with physicians and interdisciplinary teams; complete documentation using AI-assisted tools (WellSky/Kinnser); manage a flexible schedule with 1:1 patient care in home settings; required to work in the Ft Myers, FL area with travel to patients' homes; Florida LPN license and CPR certification required.
Required Qualifications
- Active, unencumbered Florida Licensed Practical Nurse (LPN) license
- Valid driver’s license and reliable transportation
- CPR certification
- Home health experience preferred but not required
