Job Summary
Provide treatment and health care procedures ordered by a physician, monitor health and medical conditions, and advise the supervisor of all concerns related to nursing services. Create and distribute medical reports, develop Individual Support Plans with an interdisciplinary team, and train employees on assigned individuals' medical needs. Ensure program documentation is completed accurately, maintain medication logs, and build a support network by developing relationships with community members and families. Participate in social activities, circles of support for personal futures planning, and follow strict OSHA procedures. Hours and weekly schedules may change due to business needs, including evening and weekend hours, with occasional overtime and on-call requirements. This role supports individuals with disabilities in residential, day, and community settings within a national non-profit agency.
Required Qualifications
- Bachelor's Degree or equivalent experience in nursing field
- Minimum of one year of experience providing nursing services to individuals with disabilities
- Valid nursing license
- Valid driver's license with a satisfactory driving record
- Complete all state and agency required training per state guidelines
- Ability to calculate figures related to medication dosages
- Understanding and commitment to community-based support for persons with disabilities
- Ability to exercise good judgment and remain calm in crisis situations
- Ability to establish a comfortable and supportive relationship with an individual receiving supports
- Proficient with Microsoft Office
- Ability to effectively present facts and recommendations
- Ability to effectively communicate with program participants, third-party agencies, staff and members of the community
- Must be available for evening and weekend hours
- Must be able to lift or move 50+ pounds
- Must be able to assist with wheelchair transfer of non-ambulatory individuals
- Must be able to transport individuals utilizing your own vehicle or company provided vehicles
