Licensed Practical Nurse (LPN)

Job Summary
Provide nursing care in community-based settings for individuals with disabilities, delivering care per physician orders and professional standards. Administer medications, monitor health conditions, and refer as needed. Develop and update Individual Support Plans (ISP) and Person Centered Plans (PCP) with an interdisciplinary team, including concerns, observations, and behavioral developments. Train employees on medical needs, maintain health service documentation and billable records, ensure availability of health supplies, and participate in community activities. Maintain OSHA standards for bloodborne pathogens and cooperate with inspections. Requires Bachelor’s Degree or equivalent nursing experience, valid nursing license, and a valid driver’s license; prior nursing experience with individuals with disabilities preferred. Sign-on bonus of $5,000 paid after 90 days with approval.
Required Qualifications
- Bachelor’s Degree or equivalent experience in nursing field
- Minimum of one year of experience providing nursing services to individuals with disabilities
- Valid nursing license
- Valid driver’s license with a satisfactory driving record
- Complete all state and agency required training per state guidelines
- Ability to calculate figures related to medication dosages
- Understanding and commitment to community-based support for persons with disabilities
- Ability to exercise good judgment and remain calm in crisis situations
- Ability to establish a comfortable and supportive relationship with an individual receiving supports
- Proficient with Microsoft Office
- Ability to effectively present facts and recommendations
- Ability to effectively communicate with program participants, third-party agencies, staff and members of the community
