Licensed Practical Nurse (LPN)

Admissions, discharge, and transfers of residents; medication requisitions with pharmacy and physicians; medication administration. Following infection control, emergencies, and center policies; collaborating with the interdisciplinary team to provide the best outcomes. Identify and report abnormal changes in residents' condition to proper personnel per center policy. Deliver nursing care to patients requiring long-term or rehabilitative care; collect data, make observations, and report pertinent information. Implement the patient/resident plan of care and evaluate response; maintain knowledge of documentation requirements and equipment maintenance. Maintain confidentiality regarding all resident information. Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility policies, and nursing judgment.
