Licensed Practical Nurse (LPN)

Assess, monitor, and document resident conditions while administering medications and managing care plans for patients in long-term or rehabilitative settings. Coordinate admissions, discharges, and transfers; requisition medications with pharmacy and physicians; and report abnormal changes to proper personnel per center policy.
