Licensed Practical Nurse (LPN)

Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN. Communicates with attending physicians and hospice team members to coordinate care and use community resources for patients/families. Documents services on appropriate forms and demonstrates knowledge of Medicare/Medicaid hospice regulations. Reports changes in patient condition to the RN. Requires Mississippi LPN license, CPR certification, and 2-3 years of LPN experience; hospice experience preferred.
